Announcement on MJG Firewire Initiators Quality Issues

We wanted to provide a formal announcement with regard to recent quality issues with MJG Firewire Initiators. As a large reseller of MJG products, it's critical we stand behind the quality and performance of the products we are selling.

What happened?

The chemical slurry, essential for the firing process, was not being mixed adequately. The issue manifested randomly based on the speed of the dipping process and when the chemical was added. The root cause of the issue was identified on August 11, 2023.

How is this solved going forward?

The solution is to simply perform a consistent mixing of the chemical slurry every three minutes. This is now part of standard procedure. This solution is now in place and resolved on all batches after 2023.05.17, explained further in this announcement.

How many instances of this issue have been formally reported to COBRA?

We have had 5 instances reported to COBRA which represents a fraction of a percent of shipments.

What percent of initiators could be impacted by this issue if the batch was affected?

Our worst case was 6%. However, MJG has seen upwards of 17% in an extreme case scenario. Other reports from COBRA clients have as low as 1-2% failure.

What batches of initiators were affected?

The below batches have some level of risk. All boxes of initiators have a date stamp you can reference:

If I have MJG initiators with this batch number, does this mean I will have issues?

No, however, there is some risk which unfortunately cannot be determined.

Is there any way to visually see if an initiator has any issues?

No, it's not possible to visually determine this.

If any of my MJG igniters are of the noted batch numbers, is there anything I can do?

If you have any shipments with the noted batch numbers and have not experienced any issues, the likelihood of issues developing is very low. However, if you have any concerns, your confidence in the product and your experience are extremely important. We can work with you and MJG to address this and create any solutions where necessary to ensure confidence in the product.

Should I be concerned about this issue happening again?

No, the root issue has been addressed and COBRA has worked with MJG to remove any affected units from their warehouse for any future shipments.
